This patient had a modified radical mastoidectomy for cholesteatoma. The ossicular chain was completely destroyed, except for the footplate of the stapes. A titanium total ossicular replacement prosthesis (TORP) was placed over the footplate.

The "platform" of the TORP was covered with a piece of tragal cartilage and a temporalis fascia graft. The patient's hearing improved by 40 dB.
